Service Coordination
Service Coordinators work in partnership with individuals and families to ensure access to services; to monitor satisfaction and to work towards independence for all individuals. The service coordinator works for an individual in developing an Individualized Service Plan (ISP). The ISP identifies individual needs, desired outcomes and future planning. UCP of NYC’s Service Coordination Program is nationally accredited through CARF (Commission on Accreditation of Rehabilitation Facilities). Service Coordination offers quality individualized services and supports for people with developmental disabilities so they can lead independent and fulfilling lives.
