For more than 25 years, UCP of NYC has specialized in meeting the complete health care needs of individuals with disabilities and their families. Our clinics offer medical and dental primary care, preventative planning, as well as specialty clinical and therapeutic services. Our wellness philosophy supports the active participation of the individual in every phase of the health care process. We believe that personal health is essential to feeling good, being our best and leading fulfilling lives.

We employ a staff of highly trained clinicians, therapists and assistants whose vast expertise ensures we provide exceptional quality, patient sensitive health care. From the onset of a first scheduled visit, patient care coordinators and nurse case managers work in tandem with individuals to determine their needs and navigate treatment options.  While our clinics boast state of the art, handicap accessible equipment and services, we recognize the importance of family care. UCP of NYC is proud to make health care available to individuals with disabilities and their families in a single location.

UCP of NYC’s Article 28 medical clinics have been recognized by the National Committee for Quality Assurance (NCQA) as Patient-Centered Medical Home (Level 2). The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement. We currently accept Medicare and Medicaid insurance for our clinical services. You can contact our clinics directly for a Out of Network or Self Pay fee schedule.
