Program Description
UCP/NYC offers a comprehensive range of residential services for adolescents and adults with physical and developmental disabilities. These range from intermediate care facilities for the most disabled, to supportive apartments for those requiring minimal staff assistance.

Services
Residential services are structured on the continuum of "most restrictive" environment to "least restrictive" environment. This provides every individual an opportunity to live in a home that enhances independence while supporting individual needs. It also allows for progression to less restrictive environments as the individual is ready. Specific residential services available through UCP/NYC include:

  • Intermediate Care Facilities (ICFs) are residences within the community that offer the most structure and support. Medical, nursing and clinical services are available within the home. Highly trained staff provide 24-hour supervision which includes instruction and assistance to aid each resident in achieving greater independence. Family members are considered part of the treatment team and are integral to the development of the resident's plan of care.

  • Individualized Residential Alternatives (IRAs) are residences within the community that provide supervised care, as warranted by the needs of each individual. Residents choose the living environment and services that best meet their needs. Homes range from a single studio apartment to a free standing house shared with others.

  • Supportive IRAs are for persons who have achieved a high degree of independence. This option offers less than 24-hour supervision, enabling the person to live on their own and make their own life choices.

  • Independent Living is an option for persons who have achieved independence and are capable of living on their own. For many individuals, UCP/NYC may continue to provide such supports as service coordination, in-home adaptations and technology resources.

Hours of Operation
As noted above, ICFs and IRAs provide 24-hour supervised care, with all residents required to participate in an adult day program or be competitively employed. Individuals in supportive apartments are either competitively employed, participate in a vocational program where they are paid wages, or are retired.

Eligibility
New York City residents of Manhattan, Brooklyn, the Bronx and Staten Island who are physically and developmentally disabled.

Funding and Fees
Funding for residential services varies according to the type of facility and the individual's ability to contribute to the financial support of his or her residence.

UCP/NYC's Residential Services are located in four boroughs of New York City -- Manhattan, Brooklyn, the Bronx and Staten Island -- in various neighborhoods and in various types of dwellings.
