For over 25 years, UCP/NYC has specialized in meeting the complete health care needs of individuals with disabilities and their families. Our philosophy is one which supports the active participation of the individual consumer in every phase of the health care process.

With unique services like our nurse case managers who coordinate patient care from the first date of an individual's visit, UCP/NYC's emphasis is on providing quality, patient-sensitive health care. Our highly trained staff of clinicians and assistants have years of experience in working with people with disabilities and focus particularly on the communication necessary for the most effective treatment and services. Our health care staff also understands the importance of preventive care, a major component of UCP/NYC's medical care program.

Rehabilitation Services
UCP/NYC's 50 years of experience in working with individuals with physical disabilities have made our staff recognized leaders in the rehabilitation field. The comprehensive range of rehabilitation services available include:

Adaptive Equipment Services
UCP/NYC's Adaptive Equipment Clinics — located in Manhattan, Brooklyn, the Bronx and Staten Island — assist with both the long-term needs of those with chronic disabilities as well as the short-term needs of those with temporary disabilities. Our physiatrists, occupational therapists, and physical therapists provide highly personalized services and follow-up. They can assess, prescribe, order, maintain and repair a full range of adaptive equipment and rehabilitation devices, including:

  • mobility equipment (wheelchairs, scooters, walkers)
  • seating and postioning systems
  • customized moldings for wheelchairs
  • orthotics and prothotics (braces, splints, customized body jackets)
  • augmentative communication devices
  • activities of daily living equipment
  • home environment adaptations

Nutrition Services
UCP/NYC offers nutrition screenings, interventions and counseling by a registered nutritionist in collaboration with an interdisciplinary team of primary health care practitioners. Their goals are to prevent disease, speed recovery and maintain wellness by designing a health management program that meets the individual's outcome objectives, particularly for those who have chronic health issues. Services include:

  • health promotion
  • disease management
  • individual counseling
  • weight control groups for children and adults with eating disorders

Scheduling an Appointment
UCP/NYC operates three principal Health Care Centers — one in Manhattan, one in Brooklyn and one in the Bronx — which serve residents of the five boroughs of New York City, as well as the surrounding metropolitan area. Satellite facilities in the Bronx and Staten Island offer additional locations for access to some health care services. All facilities are completely wheelchair accessible. UCP/NYC's Health Care Centers are generally open weekdays from 9:00 a.m. to 9:00 p.m. and Saturdays from 9:00 a.m. to 1:00 p.m. Please call in advance to schedule an appointment.

Fees
UCP/NYC accepts Medicaid, Medicare and private health insurance. Our staff will gladly assist in completing all necessary forms.
