Program Description
UCP/NYC's Service Coordination Program assists
individuals with a developmental disability
in accessing needed services. A personal "service
coordinator" works with the consumer and
his/her family to develop an individualized
plan for meeting the variety of needs that the
individual might have, including educational,
vocational, recreational, residential, medical,
social and financial.
Services
Trained coordinators function essentially as
services brokers, arranging for service delivery
from a variety of government entities (OMRDD,
Board of Education, etc.) and private nonprofit
organizations. Ongoing services include:
- advocacy
- linkage and referral
- outreach
- crisis intervention
- follow-up and monitoring
Hours of Operation
The Service Coordination may be contacted Monday
through Friday from 9:00 a.m. to 5:00 p.m.

Eligibility
Any individual with a developmental disability
who lives in the Bronx, Brooklyn, Manhattan
or Staten Island. Please note that you do not
have to be a current UCP/NYC program participant
in order to receive case management services.
Funding
Service Coordination is funded by the New York
State Office of Mental Retardation & Developmental
Disabilities under the Medicaid Waiver program.
The program is also supplemented by private
contributions to UCP/NYC. There are no fees
associated with utilizing the program.
Location and Travel Information
Service Coordination is located within UCP/NYC's
Family Support Service division at 120 East
23rd Street, 5th Floor, in Manhattan. The cross
streets are Park Avenue South and Lexington
Avenue. This location is easily accessible by
subway and bus. In addition, service coordinators
can travel to an individual's home to meet with
the consumer and his/her family. In addition,
there is a local Services Coordination Office
in Brooklyn.
