Program Description
UCP/NYC's Day Habilitation Program focuses on
skills development, increased self-sufficiency,
volunteer opportunities, and community integration
for adults ages 21 and older who are developmentally
and physically disabled. Perhaps the most important
components of Day Habilitation are community
integration (participants spend up to 50% of
their time each week engaged in activities in
the community) and participant choice (participants
work with staff to select the "mix "of
activities they will enjoy).
Services
Day Habilitation services focus on activities
in eight core areas:
- education
- entrepreneurship
- consumerism
- volunteerism
- cultural/aesthetic
- physical fitness
- spirituality
- seniors' activities (for those 60 and older)
In addition, travel training is an important
part of the program for many participants. Medical,
dental and rehabilitative services are available
through the Article 28 clinic located on the
same premises, including primary medical care,
specialty care, speech therapy, occupational
therapy, physical therapy, social work, nursing,
psychology, psychiatry, nutritional counseling,
orthotics, audiology, adaptive equipment and
assistive technology.
Hours of Operation
Day Habilitation programs operate Monday through
Friday year-round. Starting and ending times
vary according to the individual program, but
generally range from 9:00 a.m. to 4:00 p.m.
In addition, Manhattan offers an Evening Habilitation
program between the hours of 4:00 - 9:00 p.m.

Eligibility
Eligible applicants include any adult 21 years
of age or older who is developmentally disabled
and determined to benefit from the level of
adult day program provided by Day Habilitation.
Individuals must also be eligible for Medicaid
Waiver services.
Funding
Program and transportation funding is provided
by the New York State Office of Mental Retardation
& Developmental Disabilities (OMRDD) through
the Medicaid Waiver program for individuals
who are Medicaid Waiver eligible. Supplemental
support is provided through private contributions
to UCP/NYC. There is no direct cost to participants
who attend this program.
Location/How to Apply
UCP/NYC operates two Day Habilitation programs
based at the following locations. All are accessible
to nearby public transportation. Door-to-door
transportation is also provided for those unable
to utilize public transportation. Please call
